1. It will open a new application form where you have to select District, Block/ Municipality, Gram Panchayat/ Ward, Pin Code, Post Office, Police Station, Flat/ Premises Number, Street/ Lane Number, etc.
2. You will also have to fill DOB, EPIC number, Aadhar number, PAN
3. Enter the address to which the card should be delivered
4. Next, click on the Show Members button to check the details. If you want to add more, you can click on Add Member Details.
5. Submit the application and you will receive an application number that can be used to track the form.
